29 CFR 1926.403(i)(2)(i): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by cabinets or other forms of enclosures, nor by any of the means listed in subparagraphs (A) through (D) of this paragraph:    (a) Location - Basement of 49 Queenstown Street, Devens, MA:  On or about 5/5/15, employees were not protected from electric shock and burns where the main electric panel operating at 208 volts was missing its cover. The employer failed to install a cover prior to having employees work in and around the unguarded equipment.

29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:  (a) Location - Basement of 49 Queenstown Street, Devens, MA: On or about 5/5/15, employees were not protected from electric shock and burns where live electric work was conducted in the main electric panel where the electric service for a subpanel was added to a 240 volt circuit breaker. The circuit breaker was turned to the off position when the work was done however, the power to the panel which operated at 208 volts was live and live parts were accessible by employees with their hands and hand tools.

29 CFR 1926.405(a)(2)(ii)(E): Lamps used in temporary wiring for general illumination were not protected from accidental contact or breakage:    (a) Location - First floor, 49 Queenstown Street, Devens, MA:  On or about 5/5/15, employees were not protected from electric shock where industrial compact fluorescent light fixtures were not completely guarded. The employer failed to install temporary lighting fixtures that were manufactured with complete guards.    (b) Location - 49 Queenstown Street, Devens, MA:  On or about 5/5/15, employees were not protected from electric shock where temporary light fixtures using incandescent light bulbs were found with missing plastic cages on multiple floors.
