29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:  Along the center of an excavation adjacent to a highly trafficked public road, on or about May 07, 2015 employees were exposed to caught-in hazards while doing an installation of a concrete storm sewer line measuring approximately 2 feet 6 inches in diameter by 8 feet in length working inside an excavation measuring approximately 10 feet deep by 4 feet wide by 24 feet long without having a safe means of egress.
