At the workplace located at 33 N. Third Street, Columbus, Ohio, employees were exposed to a fall hazard when manually opening the freight elevator doors on the first floor, using the key tool, while the freight elevator was positioned at a floor above the first level.  On May 6, 2015, an employee fell approximately 13 feet into the freight elevator hoistway, from the first floor lobby level to the elevator pit resulting in serious injury.
