29 CFR 1903.19(d)(1): The employer must submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required:   a. In regards to citation 1, item 1, issued on July 15, 2015, the employer has failed to submit abatement certification within ten days of the abatement date, June 9, 2016.  The violation read as follows:    29 CFR 1910.23(c)(1): Every open-sided floor or platform four feet or more above adjacent floor or ground level shall be guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder.   a.  At the workplace located at 33 N. Third Street, Columbus, Ohio, employees were exposed to a fall hazard when manually opening the freight elevator doors on the first floor, using the key tool, while the freight elevator was positioned at a floor above the first level.  On May 6, 2015, an employee fell approximately 13 feet into the freight elevator hoistway, from the first floor lobby level to the elevator pit resulting in serious injury.
