29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.   a.  On or about May 8, 2015, located in the machining room, employees operated a Brown & Sharpe screw machine, model number 3/4, serial number 542-00-6602, without any guarding to prevent employees from making contact with the point of operation and/or ingoing nip points, thereby exposing the employees to an amputation and/or laceration hazard.  b.  On or about May 8, 2015, located in the machining room, employees operated a Brown & Sharpe screw machine, model number 3/4, serial number 542-00-6493, without any guarding to prevent employees from making contact with the point of operation and/or ingoing nip points, thereby exposing the employees to an amputation and/or laceration hazard.  c.  On or about May 8, 2015, located in the machining room, employees operated a Brown & Sharpe screw machine, model number 1-5/8, serial number 542-2-8099, without any guarding to prevent employees from making contact with the point of operation and/or ingoing nip points, thereby exposing the employees to an amputation and/or laceration hazard.  d.  On or about May 8, 2015, located in the machining room, employees operated a Brown & Sharpe screw machine, model number 3/4, serial number 542-2-4836, without any guarding to prevent employees from making contact with the point of operation and/or ingoing nip points, thereby exposing the employees to an amputation and/or laceration hazard.  e.  On or about May 8, 2015, located in the machining room, employees operated a Brown & Sharpe screw machine, model number 3/4, serial number 542-2-7598, without any guarding to prevent employees from making contact with the point of operation and/or ingoing nip points, thereby exposing the employees to an amputation and/or laceration hazard.  f.  On or about May 8, 2015, located in the machining room, employees operated a Brown & Sharpe screw machine, model number 1-5/8, serial number 542-2-7162, without any guarding to prevent employees from making contact with the point of operation and/or ingoing nip points, thereby exposing the employees to an amputation and/or laceration hazard.  g.  On or about May 8, 2015, located in the machining room, employees operated a Brown & Sharpe screw machine, model number 3/4, serial number 542-00-9451, without any guarding to prevent employees from making contact with the point of operation and/or ingoing nip points, thereby exposing the employees to an amputation and/or laceration hazard.  h.  On or about May 8, 2015, located in the machining room, employees operated a Brown & Sharpe screw machine, model number 1/2, serial number 542-00-9212, without any guarding to prevent employees from making contact with the point of operation and/or ingoing nip points, thereby exposing the employees to an amputation and/or laceration hazard.  i.  On or about May 8, 2015, located in the machining room, employees operated Miyano CNC lathe model number LE-22, serial number L220056, without adequate guarding in order to prevent the employees from an amputation and/or laceration hazard.   j.  On or about May 8, 2015, located in the machining room, employees operated Miyano CNC lathe model number LE-22, serial number L220383, without adequate guarding in order to prevent the employees from an amputation and/or laceration hazard.  k.  On or about May 8, 2015, located in the machining room, employees operated Miyano CNC lathe model number BNC-34S, serial number BN3002SU, without adequate guarding in order to prevent the employees from an amputation and/or laceration hazard.  l.  On or about May 8, 2015, located in the machining room, employees operated Miyano CNC lathe model number BNC-34C, serial number BN3051SU, without adequate guarding in order to prevent the employees from an amputation and/or laceration hazard.

29 CFR 1910.215(a)(4): Work rests. On offhand grinding machines, work rests shall be used to support the work. They shall be of rigid construction and designed to be adjustable to compensate for wheel wear. Work rests shall be kept adjusted closely to the wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage. The work rest shall be securely clamped after each adjustment. The adjustment shall not be made with the wheel in motion.  a.  On or about May 8, 2015, located in the machining room, employees operated a 6 inch DeWalt bench grinder, model number DW756, serial number 2011 50-YL64380, in order to sharpen machine tooling by hand while the work rest was greater than one-eighth inch, thereby exposing the employees to a struck-by, amputation, and/or laceration hazard.   b.  On or about May 8, 2015, located in the machining room, employees operated an 8 inch Craftsman bench grinder in order to sharpen machine tooling by hand while there was no work rest installed to prevent employee exposure to a stuck-by, amputation, and/or laceration hazard.

29 CFR 1910.215(b)(9): Exposure adjustment. Safety guards of the types described in Subparagraphs (3) and (4) of this paragraph, where the operator stands in front of the opening, shall be constructed so that the peripheral protecting member can be adjusted to the constantly decreasing diameter of the wheel. The maximum angular exposure above the horizontal plane of the wheel spindle as specified in paragraphs (b)(3) and (4) of this section shall never be exceeded, and the distance between the wheel periphery and the adjustable tongue or the end of the peripheral member at the top shall never exceed one-fourth inch. (See Figures O-18, O-19, O-20, O-21, O-22, and O-23.)  a.  On or about May 8, 2015, located in the machining room, employees operated a 6 inch DeWalt bench grinder, model number DW756, serial number 2011 50-YL64380, in order to sharpen machine tooling by hand while the adjustable tongue guard was missing, thereby exposing the employees to a struck-by hazard if the grinding wheel would explode.    b.  On or about May 8, 2015, located in the machining room, employees operated an 8 inch Craftsman bench grinder in order to sharpen machine tooling by hand while the adjustable tongue guard was missing, thereby exposing the employees to a struck-by hazard if the grinding wheel would explode.

29 CFR 1910.219(d)(1): Guarding. Pulleys, any parts of which are seven (7) feet or less from the floor or working platform, shall be guarded in accordance with the standards specified in paragraphs (m) and (o) of this section. Pulleys serving as balance wheels (e.g., punch presses) on which the point of contact between belt and pulley is more than six feet six inches (6 ft. 6 in.) from the floor or platform may be guarded with a disk covering the spokes.  a.  Located in the workplace, the employer did not ensure that guarding was installed over a pulley located on the back of a Craftsman table saw, thereby exposing the employees to the ingoing nip point where the belt met the pulley.

29 CFR 1910.219(e)(3)(i): Vertical and inclined belts shall be enclosed by a guard conforming to standards in paragraphs (m) and (o) of this section.  a.  Located in the workplace, the employer did not ensure that guarding was installed over a belt located on the back of a Craftsman table saw, thereby exposing the employees to the ingoing nip point where the belt met the pulley.
