29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above a lower level was not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   A) Job site, on or about May 5, 2015, employee installing floor joist for a second floor were not protected from fall hazards, exposing the employees to falls heights of approximately 9 and 10 feet.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access:  A) Job site, on or about May 5, 2015, employees used a 6 foot stepladder to access a level that was 9 feet high and the employer did not provide a ladder capable of providing access to the upper landing with 3 feet of ladder rail extension above the upper landing, exposing employees to a 9 foot fall hazard.
