29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    a) Building 5, Mallard Court, Flemington, NJ 08822. Employee of JHI Construction, while working under the direct supervision of the Fox Chase Superintendent was exposed to struck by hazards when working at ground level under active roofing work while not wearing any head protection, i.e. hard hats.      b) Building 6, Mallard Court, Flemington, NJ 08822. Employee of Esperanza Contractors LLC, while working under the direct supervision of the Fox Chase Superintendent was exposed to struck by hazards when working at ground level under active roofing work while not wearing any head protection, i.e. hard hats.      Conditions occurred on or about 05/11/15.

Employee of JHI Construction, while working under the direct supervision of the Fox Chase Superintendent was exposed to eye and face injuries from flying debris when operating a pneumatic nail gun and not wearing any eye and or face protection.    b) Building 6, Mallard Court, Flemington, NJ 08822. Employee of Esperanza Contractors LLC, while working under the direct supervision of the Fox Chase Superintendent was exposed to eye and face injuries from flying debris when operating a pneumatic nail gun and not wearing any eye and or face protection.       Condition occurred on or about 05/11/15.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above the lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of 29 CFR 1926.502(k) developed and implemented:      a) Building 5, Mallard Court, Flemington, NJ 08822. Employees of JHI Construction, while working under the direct supervision of the Fox Chase Superintendent were exposed to fall hazards when placing and installing new roofing materials with no fall protection systems in place.    b) Building 6, Mallard Court, Flemington, NJ 08822. Employee of Esperanza Contractors LLC, while working under the direct supervision of the Fox Chase Superintendent was exposed to fall hazards when placing and installing new roofing materials with no fall protection system in place.        Condition occurred on or about 05/11/15.
