29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about May 05, 2015, employees were exposed to fall hazards greater than 6 feet to the ground level below, when working on the roof of a residential home not wearing safety harnesses for fall protection, while installing the felt paper to the roof decking with a roof pitch of 10/12.

29 CFR 1926.1052(c)(1): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail and one stairrail system along each unprotected side or edge:  On or about May 05, 2015, employees were exposed to falls greater the 6 feet to the concrete floor below, when climbing the stairway of the residential home, which was not provided with a guard rail system to the second floor, while using the stair way for access to the roof.
