29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   On or about May 05, 2015, employees were exposed to struck-by hazards from items such as but not limited to: hammers, when not wearing protective helmets, while other employees were working above them on the roof installing felt paper to the roof decking.

29 CFR 1926.1052(c)(1): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail and one stairrail system along each unprotected side or edge:  On or about May 05, 2015, employees were exposed to falls greater the 6 feet to the concrete floor below, when climbing the stairway of the residential home, which was not provided with a guard rail system to the second floor, while installing the residential home windows.
