29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    On or about May 06, 2015, employees were exposed to fall hazards greater than 6 feet to the ground level below, when working with no fall protection on the ceiling joist of the residential home , while installing the ceiling bracing and first floor decking.

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided:  On or about May 06, 2015, employees were exposed to falls greater the 6 feet to the concrete floor below, when climbing the interior and exterior wall bracing of the residential home, while accessing the first floor ceiling joist and installing the floor decking.
