29 CFR 1926.501(b)(2)(i): Each employee who was constructing a leading edge 6 feet or more above lower levels was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems:   (a)  114 Sibley Avenue, Ardmore, PA - On or about May 13, 2015, employees constructing the leading edge of a Peri SKYDECK Aluminum Slab Formwork System were not protected from falls of approximately 20-22 feet to ground, in that the employee dropping the panels into the formwork was not protected from falls by the installation of guardrails, safety net systems, or personal fall arrest systems.  Note:  If the employer could demonstrate that it was infeasible or created a greater hazard to use these systems, a fall protection plan meeting the requirements of paragraph (k) of 1926.502 should have been developed and implemented.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  (a)  114 Sibley Avenue, Ardmore, PA - On or about May 13, 2015, employees working on a Peri SKYDECK Aluminum Slab Formwork System were not protected from falls of approximately 9.5 feet to the next lower level, in that there was an approximately 5 foot space between the guardrail and access ladder which was not protected.
