29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval. Capacity, operation, and maintenance insturction plates, tags or decals of the powered industrial truck were not changed accordingly:    a.) Ridg-U-Rak/120 South Lake Street Northeast, PA:  Forklift extension attachments were utilized on forklifts that included but were not limited to the Yale (Serial No. 21368K/Lift Truck GLC)&VXNVSE).  Manufactures approval for the forklift extensions was not obtained.  In addition, the forklifts capacity plate was not modified to reflect the forklift extension attachments effect on the forklifts capacity.

29 CFR 1910.178(a)(5): When the truck was equipped with front-end attachments other than factory installed attachments, the user did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered:    a.) Ridg-U-Rak/120 South Lake Street Northeast, PA:  On or about May 7, 2015, forklift extension attachments were utilized on forklifts that included but were not limited to the Yale (Serial No. 21368K/Lift Truck GLC)&VXNVSE).  Manufactures approval for the forklift extensions was not obtained.  In addition, the forklifts capacity plate was not modified to reflect the forklift extension attachments effect on the forklifts capacity.
