OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that the employees were exposed to fall and struck-by hazards:   a. 1601 Route 22 West, Watchung, NJ -   Employees engaged in merchandise inventory activities worked from a personnel cage/basket attached to a Crown, Model RC5515-30 Forklift, that was not properly secured to the forks and lifting carriage using the fork locking straps and where employees used web straps to secure the basket to the carriage.   The web strapping was two straps tied together in knots and tied around the carriage to prevent the basket from becoming dislodged while elevated.    Violation observed on or about April 7, 2015
