29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:  (a) Warehouse: Without the manufacturer's approval, the employer attached the top of a roller table to the forks of the lift so the lift could be utilized as part of a conveyor system while they waited on the new parts to arrive, observed on or about 5/8/15.  NOTE:  THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM, FAILURE TO DO SO WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 AS PER 29 CFR 1903.19.
