29 CFR 1926.451(f)(6): Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, could have come closer than the minimum distances established for specific voltages as set forth in the table in subparagraph (f)(6).    (a) The employer did not ensure that employees, using an aerial lift to access the roof of a commercial building, maintained the necessary clearances from power lines, exposing employees to electrical shock hazards.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    (a) The employer did not ensure that employees were using fall protection when conducting carpentry work on a commercial building.
