29 CFR 1910.303(b)(7)(iv): There were damaged parts that may adversely affect safe operation or mechanical strength of electric equipment, such as parts that were broken, bent, cut or deteriorated by corrosion, chemical action, or overheating: (a) On or about Friday, May 15, 2015, in the administrative office, the electrical plug that controls the chillers was pulled from the conductor. Employees were exposed to live electrical parts which could lead to electrical burns or shock.

29 CFR 1910.305(j)(1)(i): Fixtures, lamp holders, lamps, rosettes, and receptacles had live parts normally exposed to employee contact: (a) On or about Friday, May 15, 2015, at the warehouse entrance live parts were exposed on a receptacle due to the faceplate being too small. Employees were exposed to live energized parts which could lead to electrical burns or shock.
