On or about May 13, 2015, at 602 Madison Park Drive, Grayson, Georgia:  An employee performing roofing work from the ground was exposed to struck-by hazards from falling shingle bundles and the tools of the employees working above him when he did not use head protection.

29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head:  On or about May 13, 2015, at 602 Madison Park Drive, Grayson, Georgia:  An employee performing roofing work was exposed to a fall hazard of approximately 25-feet and greater when he worked from an approximately 10/12 slope roof while using a rock climbing harness with an attachment at the front of the employee's waist.
