Cecil St., Bonduel, WI 54107, were not protected from falling approximately 9 feet to the ground on a high slope roof by the use of a fall protection system as outlined above.

Cecil St., Bonduel, WI 54107, the employer did not ensure that the portable fiberglass ladder was extended 3 feet to access upper landing surfaces and was secured at the top of the ladder to prevent displacement.

29 CFR 1926.20(b)(1): The employer did not initiate and maintain an accident prevention program as may be necessary to comply with the General safety and health provisions covered in 29 CFR 1926.20.  a)  On or about May 5, 2015, during roofing work, 340 S. Cecil St., Bonduel, WI 54107, the employer had not developed and implemented a safety and health program to ensure that hazards that employees were exposed to, including fall hazards greater than 6 feet and the proper use of ladders on construction sites, were recognized and the proper procedures were followed in order to minimize those hazards.
