29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems:  On or about May 15, 2015, at 6747 Highway 61, St. Francisville, LA, the employer did not ensure that employees conducting roofing work at a height of approximately 9 feet were provided a means of fall protection.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:  On or about 05/14/2015, at 6747 Highway 61, St. Franicsville, LA, employees were using portable ladders to access a  commercial roof of a restaurant  that was not secured at a height of approximately 9 feet to the ground.
