Shoot-A-Way, Inc. - Upper Sandusky, Ohio: On or about May 5, 2015, an energy control program was not developed and implemented.  Employees that perform die setting activities on equipment such as, but not limited to, the mechanical power press are exposed to amputation and crush by hazards in that energy control is not implemented.
