On or about 05/18/15 located at 4261 Tacoma Trace, Suwanee, GA:  Employees were observed installing 2x4 walls on to the 2nd floor of a apartment complex that was under construction with a pneumatic nail gun without safety glasses.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired.  On or about 05/18/15 located at 4261 Tacoma Trace, Suwanee GA: Employees were exposed to fall hazards, while performing framing work, employees were using a portable extension ladder to gain access to the 2nd floor, and the ladder was damaged on the right foot with structural defect.

The employer did not ensure that fall protection was used by the employees while exposed to fall hazards of approximately up to 10 feet.
