29 CFR 1910.215(a)(3): Grinding machine(s) were not equipped with flanges in accordance with 29 CFR 1910.215(c)(a): On or about May 19, 2015, the Sear Craftsman bench grinder, serial number 811600302, did not have a side flange to guard the abrasive wheel and exposed the employees to getting struck by pieces of the grinding wheel.
