29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest.  A. On or about April 22, 2015 Three employees of a 4:12 pitch roof were nailing in roofing shingles and not protected from falling by guardrail systems, safety net systems, personal fall arrest gear, the employees were exposed to a 40-foot fall hazard.
