29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilivered end.    a) On May 12, 2015 and at times prior thereto: east-side area of home: employees were working on a pump-jack scaffold whose working platform extended 36 inches beyond the scaffold's horizontal support thereby exposing employees to a fall of 12-15 feet.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  a)  On May 12, 2015 and at times prior thereto: front area/south-side and eastside of home: employees were working on a pump-jack scaffold without fall protection and were exposed to fall of approximately 12-15 feet while installing the vinyl siding.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.    a)  On May 12, 2015 and at times prior thereto; side of building; employees were using an extension ladder to access the pump-jack scaffold that was set an estimated 3 feet below the scaffold platform thereby exposing employees to a 12-15 foot fall.
