29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a) On or about May 21, 2015 - 265 Golden Delicious Drive, Oxford, AL, employees painting at approximately 11 feet 6 inches above the ground were not protected from falls.

29 CFR 1926.1053(b)(1): Where a portable ladders was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:   a) On or about May 21, 2015 - 265 Golden Delicious Drive, Oxford, AL, employees used a metal extension ladder to access a landing surface that did not extend 3 feet above the landing surface and was not tied off to prevent deflecting.
