Production Area:  On, or about, May 20, 2015, the point of operation on the Challenge Paper Drilling Machine Style RH, No. 31883, was not provided with safeguarding.  The trimming area was not guarded and the foot pedal was not guarded to prevent accidental contact.  B.  Production Area:  On, or about, May 20, 2015, the foot pedal provided on the Challenge Paper Drilling Machine Style EH-3, No. 29662 was not guarded to prevent accidental contact.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:  Production Area:  On, or about, May 20, 2015, in the following locations, flexible cords were used in lieu of permanent wiring:  A.  Legend Haum Folder:  An extension cord was used to energize the replacement counter unit installed on the machine.   B.  Challenge Paper Drilling Machine, Style RH, No. 31883:  The machine was energized through a yellow extension cord.
