29 CFR 1926.451(g)(4)(iii): When midrails, screens, mesh, intermediate vertical members, solid panels, or equivalent structural members are used, they were not installed between the top edge of the guardrail system and the scaffold platform.  Location: building exterior On or about 5/11/15, and employee was exposed to a 19 foot fall hazard while working in a scissor lift without the mid rail chain connected.
