Employee working on a driveway adjacent to highway I65 was exposed to struck-by a vehicle hazards from passing vehicles. The following deficiencies were noted:   1. Advance Warning signs were not placed at the worksite.  2. Transition and Activity Area were not provided to road users of work activity.  3. Temporary highway or alignment were not placed around the work area.  4. Pavement markings were not used as a secondary method to alert road users or work activity.
