29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: On or about 30 April, 2015 at the above addressed jobsite, employees were working from the roof of a commercial building under renovation without the use of fall protection, exposing the employee to a fall hazard of up to approximately 12 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about 30 April 2015, employees were accessing the working platform of the roof deck with a metal extension ladder that did not extend at least 3 feet above the landing, exposing the employees to a fall hazard of approximately 12 feet.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards: On or about 30 April 2015, employees were accessing the working platform of the roof deck with a metal extension ladder, without being trained on how to use the ladder, exposing the employees to a fall hazard of approximately 12 feet.
