29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On the roof of a commercial building, North side, on or about May 27, 2015, employees were exposed to a fall hazard of 11 feet 8 inches in height to unpaved ground level below when using a portable ladder that was not 3 feet above the upper landing surface to gain access to a roof.
