29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above the lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of 29 CFR 1926.502(k) developed and implemented: a) 42/44 Woehrle Avenue, Staten Island, NY 10312. Employees were exposed to fall hazards while working on and from a roof installing new roofing materials with no fall protection systems in place. Condition occurred on or about 05/28/15

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: a) 42/44 Woehrle Avenue, Staten Island, NY 10312. Employees were exposed to fall hazards while using a Werner portable ladder to access the roof when it did not extend three feet above the landing. Condition occurred on or about 05/28/15.
