29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:      a) 16 Old Rte. 9 W, Fishkill, NY - On or about May 28th, 2015 - Employees were exposed to fall hazards while using an aerial lift.  A personal fall arrest system was not worn and/or a lanyard was not attached to the boom or basket when working from the aerial lift.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.      a) 16 Old Rte. 9 W, Fishkill, NY - On or about May 28th, 2015 - Employees were exposed to fall hazards of approximately 50 feet while constructing a tower on the top level.  The employees were not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    b) 16 Old Rte. 9 W, Fishkill, NY - On or about May 28th, 2015 - Employees were exposed to fall hazards of approximately 14 feet while engaged in construction work next to a floor opening.  The employees were not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.
