29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification:  a) Construction site: On May 27, 2015 and at time prior, As the controlling employer, JE Hurley Construction Inc. did not ensure it's sub contractor, CVK Electric, LLC, protected their employees in that employees were exposed to shock hazards while using a 120V receptacle designed to be permanently mounted on a wall as a temporary power source.  b) Construction site: On May 27, 2015 and at time prior, employees were exposed to shock hazards while using a 120V receptacle designed to be permanently mounted on a wall as a temporary mounted power source.

29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupers for personal protection:  a) Construction site: On May 27, 2015 and at time prior, As the controlling employer, JE Hurley Construction Inc. did not ensure it's sub contractor, CVK Electric, LLC, protected their employees in that employees were exposed to shock hazards while using an electrical box to energize equipment such as but not limited to a fan and monitor.  b) Construction site: On May 27, 2015 and at time prior, employees were exposed to shock hazards while using an electrical box to energize equipment such as but not limited to a fan and monitor.
