Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:    On or about May 28, 2015, at a worksite located at 1672-1676 Pearl Road, Brunswick, OH 44212, during roofing work activities, the employer did not ensure employees were protected from fall hazards approximately 12 to 14 feet. The employer was not following the fall protection regulations for roofing activities on a low-sloped roof.
