29 CFR 1926.20(b)(2): Frequent and regular inspections of the job site, materials, and equipment to be conducted by a competent person designated by the employer was not provided by the employer who was responsible to initiate and maintain an accident prevention program: a- Building 500, Mullica Hill NJ: A competent person did not conduct frequent and regular inspections of the jobsite. A subcontractor's employee, under the direction of the company project manager, was exposed to hazards including, but not limited to fall hazards. Condition noted on or about 05/15/15. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a- Building 500, Mullica Hill NJ: A subcontractor's employee, under the direction of the company project manager, was exposed to fall hazards in excess of 8 feet to the ground surface below when performing roof shingle installation (and other related activities) from a pitched roof. This employee was not utilizing any fall protection at the time of the worksite inspection. Condition noted on or about 05/15/15. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
