29 CFR 1910.1200(e)(1): The employer did not develop, implement, and maintain at each workplace, a written hazard communication program which described how the criteria specified in 29 CFR 1910.1200 paragraphs (f), (g), and (h) for labels and other forms of warning, safety data sheets, and employee information and training would be met. (Construction Reference 1926.59):    a.) 1720 Fairmont Ave, Philadelphia, PA. - The employer did not have a hazard communication program to inform employees of the hazards associated with tile operations. This exposed the employees to numerous hazards, including, but not limited to, silica containing materials. Observed on or about 5/28/15.

The GFCI was removed, exposing the employees to electrical hazards, on or about 5/28/15.

29 CFR 1926.403(e): Splices. Conductors shall be spliced or joined with splicing devices designed for the use or by brazing, welding, or soldering with a fusible metal or alloy. Soldered splices shall first be so spliced or joined as to be mechanically and electrically secure without solder and then soldered. All splices and joints and the free ends of conductors shall be covered with an insulation equivalent to that of the conductors or with an insulating device designed for the purpose.   a.) 1720 Fairmont Ave, Philadelphia, PA. - The flexible cord used to power the DeWalt table saw was spliced and covered with duct tape that was not rated as an insulating device. This exposed the employees to electrical hazards, on or about 5/28/2015.
