(a)  At 100 McKeogh St., Saraland, AL: On June 3, 2015 and at times prior to; employees who were affixing asphalt shingles on a 4:12 pitch roof, were exposed to falls of approximately 17 feet.

29 CFR 1926.1060(a)(1)(iii): The employer did not ensure that each employee using ladders and/or stairways had been trained by a competent person in the proper construction, use, placement, and care in handling the ladders and/or stairways:  (a)  At 100 McKeogh St., Saraland, AL: On June 3, 2015 and at times prior to; employees who were affixing asphalt shingles on a 4:12 pitch roof and using a ladder to access the roof of the 2 story apartment complex, were exposed to falls of up to 17 feet while using an extension ladder that did not extended at least 3 feet above the landing and were not informed or trained to know that the ladder set-up was not correct.
