King Drive, Chicago, IL. - employees applying mortar were exposed to the hazards associated with chemicals such as, but not limited to Portland Cement. Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact: a) On or about June 2, 2015 - at the jobsite located at 7421-23 South King Drive, Chicago, IL. - employees performing tuck pointing activities were mixing mortar without the use of personal protective equipment including, but not limited to safety glasses, gloves, dust mask, and proper clothing, thereby exposing the employees to health hazards.

Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) On or about June 2, 2015 - at the jobsite located at 7421-23 South King Drive, Chicago, IL. - an employee performing masonry work from the roof of the building was not protected from falls by the use of a personal fall arrest system, thereby, exposing the employee to fall hazards of approximately thirty (30) feet. The KM Kerry Masonry & Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 977865, citation number 2, item number 1 and was affirmed as a final order on July 24, 2014, with respect to a workplace located at 17560 S. Ridgeland Road, Tinley Park, IL. 60477. The Kerry Masonry & Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(10), which was contained in OSHA inspection number 961762, citation number 1, item number 1 and was affirmed as a final order on April 11. 2014, with respect to a workplace located at 9109 S. Western Avenue, Chicago, IL. 60643.
