29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports as follows: a. Employees were working off of the sidebrackets and had not fully decked any portion of the scaffold and were thereby exposed to a fall hazard of 10 feet.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as used as a means of access. a. Employees were observed climbing the crossbraces to gain access to the scaffold, thereby exposing the employees to a 10 foot fall hazard.

29 CFR 1926.1051(a): A stairway or ladder shall be provided at all personnel points of access where there is a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist is provided. a. The ladder that was built into the scaffold was raised to a height of 30 inches without a step or other ladder to access, thereby exposing employees to a fall hazard of 2.5 feet.
