29 CFR 1926.453(b)(2)(v):    A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:    (a) On or about June 3, 2015 at worksite 501 Conley, Columbia, Missouri- Employees engaged in window installations activities on an exterior wall and were exposed to falls of approximately 14 feet to the surface below. The employees were working from an elevated Genie S-125 aerial lift without an attached fall protection lanyard.     Abatement certification for this violation is not required.

29 CFR 1926.501(b)(1):  Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   (a)  On June 3, 2015, at worksite 501 Conley, Missouri-. employees were exposed to serious hazards, in that the unprotected sides and edges of the elevated work area was not provided with guardrails or any other means of fall protection. The employees were exposed to an approximate 20 foot fall to the surface below.    Abatement certification for this violation is not required.
