(a) At the site: On May 22, 2015 and at times prior thereto employees that erected the scaffold did not have training or experience to perform scaffold erection.  Note: Abatement certification IS required for this item.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  (a) East side of structure: On May 22, 2015 employees engaged in work from the top platform of a scaffold were not adequately protected from fall hazards of 13.9 feet onto packed earth.   Note: Abatement certification IS required for this item.
