29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about June 4, 2015, at the above addressed job site, employees were exposed to a fall hazard while using a portable extension ladder that did not extend 36 inches above the upper landing.   PLEASE NOTE THAT DOCUMENTATION AND CERTIFICATION OF ABATEMENT IS REQUIRED.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On or about June 4, 2015, at the above addressed job site, employees were exposed to a fall hazard of about 26 feet while placing concrete slabs on elevated surface, without any means of conventional fall protection.    PLEASE NOTE THAT DOCUMENTATION AND CERTIFICATION OF ABATEMENT IS REQUIRED.  Waterview Precast Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 314263849, citation number 2, item number 3, and was affirmed as a final order on January 25, 2011, with respect to a workplace located at 303 Sw 9th Street, Fort Lauderdale , FL 33315.
