29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:      (a) On or about May 15th, 2015, employees were exposed to possible injuries, including amputations, while maintaining and servicing various machines including, but not necessarily limited to, the Prism K80CD and the Titan 265 paper shears, in that the employer had not developed or implemented an energy control procedure as required by standard.      Abatement certification is required for this Item.  Use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.    NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1904.29(a): The employer did not use an OSHA 300, 301 and 300A Form or equivalent:   (a) On or about May 15th, 2015, the employer was unable to produce a copy of their OSHA-300 Log for 2013, and was unable to produce copies of their OSHA-300A Summaries for 2012, 2013, or 2014.
