29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   a) 180 Rose Avenue, Staten Island, NY: Employee was observed performing ground operations, with overhead roofing operations being performed, without a protective helmet, exposing him to struck-by hazards.    Condition observed on or about June 4, 2015.

1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used:  A) 180 Rose Avenue, Staten Island, NY: Employees were exposed to fall hazards of approximately 20' from the working level to the ground below when a ladder was not provided for safe access to a ladder jack scaffold.  Condition observed on or about June 4, 2015.

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:  a) 180 Rose Avenue, Staten Island, NY: Employees were allowed to perform roofing operations  approximately 20' from the ground below on a ladder jack scaffold without fall protection.  Condition occurred on June 4, 2015.
