Location: 226 7th Ave 3rd floor    On or about: June 9th 2015       a) Employees used a 10 inch table saw to cut wood which was not provided with a guard.

Location: 226 7th Ave Brooklyn NY 3rd and 4th floors     On or about: June 9th 2015      a) Energized electrical panels were not provided with covers.  Employees worked and walked in area carrying tools and materials near energized panels.
