29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:  JOB SITE: 1901 Broadfield Court, Chesterfield, MO 63017, on or about 6/9/2015, at this residential structure, workers were on top of the roof, operating pneumatic nail guns without any form of eye or face protection.  The workers were exposed to a struck-by hazard.  The JOSE DEL CID ROOFING COMPANY was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 950206, citation number 2, item number 2, issued on 11/29/2013, and was affirmed as a final order on 12/23/2013, with respect to a workplace located at 924 Legends View Drive Eureka, MO 63025.  The JOSE DEL CID ROOFING COMPANY was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 467759, citation number 1, item number 3, issued on 6/20/2012 and was affirmed as a final order on 08/03/12, with respect to a workplace located at 8826 Burton Avenue, Overland, MO 63114.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measures:  JOB SITE: 1901 Broadfiled Court, Chesterfiled, MO 63017, on or about 6/9/2015, at this residential structure, workers were observed on top of a 6:12 pitch roof, approximately 9' 4 inches eave to ground level, actively installing new roof shingles and were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measures.  The JOSE DEL CID ROOFING COMPANY was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 467759, citation number 1, item number 5, issued on 6/20/2012, and was affirmed as a final order on 08/03/12, with respect to a workplace located at 8826 Burton Avenue, Overland, MO 63114.
