29 CFR 1910.252(a)(1)(i): When the object to be welded or cut could not be readily moved, all movable fire hazards in the vicinity were not taken to a safe place:  Location: 100 Plain Street, Lowell, MA  On or about 6/5/15, an employee entered a manhole, cracked open a gas line to purge the gas out of the line and then cut an electrical conduit line with a DeWalt sawzall that generated a spark and caused a flash fire resulting in the employee receiving 2nd and 3rd degree burns to both arms and burns to the face and neck.

Location: 120 Plain Street, Lowell, MA - Gas attendant building  On or about 6/5/15, an employee removed the cover of the live electrical panel exposing the internal wires.
