OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:  Well site:  On or about June 3, 2015, employees were operating a rig and associated equipment during the work over of an oil/gas well with the rig guy lines being attached to an unapproved base beam anchorage system.  This exposed the employees to struck by and crushing hazards.
