29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  On or about June 10, 2015, during the installation of roofing materials on a jobsite located at 13918 W. 130th, Strongsville, Ohio 44149, the employer failed to protect its employees from a fall of approximately 14 feet by using a personal fall arrest system.
