29 CFR 1926.251(b)(1): Alloy steel sling(s) did not have permanently affixed durable identification stating size, grade, rated capacity, and manufacturer:   On or about June 08, 2015 the employer failed to inspect and ensure chain slings used had tags that indicated weight load rated capacity for chain use. The chain broke when used to lift a tank resulting in an employee struck-by a falling load.

On or about June 08, 2015 the employer failed to remove from service visually damaged chain that was being used to lift a heavy load.  This exposed an employee to a crushed-by hazard.

This exposed an employee to a struck by hazard.
