29 CFR 1918.104(a): The employer did not ensure that each affected employee wore protective footwear when working in areas where there was a danger of foot injuries due to falling or rolling objects or objects piercing the sole: Cordova Terminal, Cordova Provider: On or about June 10, 2015 and at times prior thereto, employees exposed to foot injury hazards were not required to wear protective footwear. Employees were observed wearing athletic footwear in work areas where footwear in areas where foot injuries could occur. This condition exposed employees to foot injuries.

29 CFR 1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical: Maintenance Shop: On or about June 10, 2015 and at times prior thereto, an unlabeled spray bottle container with unknown contents was found on the maintenance work bench. This condition limited employee information on the contents of the container and the proper safety and health controls necessary when using the product.
